At the end of the day, BDSM is of course never without risk, but there are forms of play where the risk is even greater and where things sometimes go wrong... Carotid Sinus Reflex is one such 'form of play'. The carotid sinus reflex is a physiological mechanism that plays an important role in regulating blood pressure. It is triggered by the carotid sinus, a small widening at the start of the internal carotid artery (arteria carotis interna) just above the point where the common carotid artery (arteria carotis communis) divides.

Consequences of CSR

It can be part of breath control or hypoxyphilia (breath restriction) and it involves compressing the carotid arteries, which can lead to Carotid Sinus Reflex (CSR), an automatic and sudden drop in blood pressure followed by loss of consciousness.

CSR is especially dangerous if the person affected has tied themselves up around the neck area and can no longer free themselves. Incidentally, deaths are generally not a direct result of the CSR itself, but rather of accompanying circumstances.

Playing with your carotid arteries - CSR - is NOT an innocent game and has led to a number of deaths in the past. These were usually cases of a strangulation game, played without a partner or proper safety measures, so that the victim could no longer free themselves. But even with good safety measures and a play partner nearby, caution is called for.

In some people, excessive stimulation of the carotid sinus can lead to a sudden, dramatic drop in blood pressure and heart rate, which can cause syncope (fainting). This is called carotid sinus syncope and can be triggered by pressure on the neck, such as wearing tight collars or making sudden head movements.
